Subtalar Arthrodesis using Fresh-Frozen Structural Allogenic Bone Graft

Abstract
Purpose: To evaluate result of subtalar fusion using cannulated screws and fresh-frozen structural allogenic bone graft in subtalar arthritis after calcaneal fracture.

Materials and Methods: Between May 2001 and May 2006, 11 patients(13 cases) who had subtalar arthritis underwent subtalar fusion with cannulated screws and fresh-frozen structural allogenic bone graft. Average age was 46 years. Radiographic analysis of the pre- and postoperative standing lateral X-rays were performed for evaluating the talocalcaneal height, calcaneal pitch angle, lateral talocalcaneal angle, and lateral talar declination angle. AOFAS Ankle-Hindfoot Scale was evaluated for clinical results.

Results: The union rate was 85%(11 of 13). The mean time to complete bone union was postop. 16.5 weeks(12~21). Radiographic analysis of the pre- and postoperative standing lateral X-rays showed an average increase of mean 8 mm in talocalcaneal height, mean 3.1 degrees in calcaneal pitch angle, mean 10 degrees in lateral talocalcaneal angle, and mean 11 degrees in lateral talar declination angle that were statistically significant(P<0.05). The change in the mean AOFAS Ankle-Hindfoot Scale score from 23¡¾17 preoperatively to 72¡¾14 postoperatively was statistically significant(P<0.05). Complications included 2 non-unions, 1 varus malalignment, 1 sural neuralgia, 1 prominent screw.

Conclusion: Subtalar fusion using cannulated screws and fresh-frozen structural allogenic bone graft in subtalar arthritis showed good results in terms of bone union, radiographic results, and functional improvement.
